Privacy Policy/Communication Preferences:
Sawtooth Physical Therapy supports and protects individual rights to privacy.
Patient information is considered confidential and protected under the Health Insurance Portability and Accountability Act of 1996 (also known as HIPPA Privacy Rules).
Every employee, observer and business associated with Sawtooth PT is required to comply with all government and company policies and procedures consistent with applicable HIPPA Laws. These policies include but are not limited to protection on disclosure of confidential and personal information. Sawtooth Physical Therapy uses password protection and encrypted electronic medical records technology to limit and secure access to patient information.
Patients have the right to opt in or out of communication between themselves and Sawtooth Physical Therapy including email, digital text and SMS messaging. Further, release of disclosure of any patient information outside of required communication for the purposes of processing billing claims and administering health care must be approved in writing by the patient or legal representative.
Patient Communication Preferences:
Authorization: As a patient of Sawtooth Physical Therapy you have the option(s) to: (a) enroll the designated mobile phone number in the Reminders and Alerts Service (b) to incur any mobile message or data charges that may be incurred by participating in the SMS/MMS Alerts Service (c) to enroll the designated email address in the Reminders and Alert Service and (d) to enroll the designated phone number(s) in the Reminders and Alert Service.
Data and Messaging Rates May Apply: We do not charge a fee for this Service, however, depending on your plan, your mobile carrier may charge you for each message we send you or that you send us. Some messages may contain links to secure documents. Accessing these documents may incur data charges. It is your responsibility to know whether your carrier will charge you data and/or messaging fees. We assume no responsibility for charges incurred by you while using the Service.
What We Will Send You: You are expressly consenting to receive text, email, and/or phone messages regarding appointment reminders, confirmations, surveys, and other communications specific to you or your child’s care.
Frequency of Messages: The frequency of messages we send will vary based on your enrollment and acceptance of different Reminder and Alert offerings. You will typically receive messages when we have information for you about your appointments or other healthcare information.
What We Expect Back From You: You must respond to an appointment text reminder to either confirm or cancel/reschedule the appointment. We do require 24 hours advanced notice of any changes needed for appointments.
Contact Information Changes: In the event you change your mobile phone number, email address, or home phone number, you should notify us immediately by calling or coming to the office to complete a change form.
Message Privacy: You understand the text messages we send may be seen by anyone with access to your mobile phone or carrier account. Accordingly, you should take steps to safeguard your phone and your text messages if you want them to remain private.
Stop Messages: You may opt out of the text alerts at any time. To stop receiving text alerts, reply STOP. After you submit a request to unsubscribe, you will receive one final text alert from our clinic confirming that you will no longer receive text alerts. No additional text alerts will be sent unless you re-activate your enrollment.
Re-Activating your Enrollment: to re-activate messaging, you must inform us. Once we have re-activated your account, you must text START to 273-71 to complete the reactivation.
Delivery Not Guaranteed: The Service may not be available in all areas at all times. The Service may not work in the event of product, software, coverage, or other changes made by your wireless carrier or changes you make to your mobile device. We are responsible for incomplete, lost , late, or misdirected messages, including (but not limited to) undelivered messages resulting from any form of filtering by your mobile carrier or service provider or otherwise.
Wireless Emergency Alerts Disclaimer: Alerts sent via SMS may not be delivered to you if your phone is not in range of a transmission site, or if sufficient network capacity is not available at a particular time. Even within a coverage area, factors beyond the control of your wireless carrier may interfere with message delivery, including the customer’s equipment, terrain, proximity to buildings, foliage, and weather. You acknowledge that urgent alerts may not be timely received and that your wireless carrier does not guarantee that alerts will be delivered.
Automatic Dialing: We may use an automatic dialing system to deliver text messages to you. By your use of the Service, you give your consent to us to use an automatic dialing system to deliver messages to the telephone or cell phone number that you have provided to transmit these text messages.
We reserve the right to modify these Reminders and Alert Terms, or any part thereof, or add or remove terms at any time. Such modification, additions or deletions will be effective immediately upon posting. Your receipt of texts after such posting shall be deemed to constitute acceptance by you of such modifications, additions or deletions.
